Ventilation holes are only available for foam mattresses.
All mattresses are breathable without ventilation holes, but to have the holes encourages airflow to circulate throughout the mattress and around your child.
The only disadvantage to having ventilation holes would be that if your child is heavily sick or heavily soils the mattress and this then soaks through the cover and into one of the holes it may be difficult to clean.
